Biosimilar Interchangeability Labeling: What FDA's Purple Book Demands from Your Audit Program

FDA's Purple Book determines how biosimilar labels must convey interchangeability status. Learn the five labeling errors auditors catch — and how AI tools automate the review.

There’s a version of this problem playing out right now in at least a dozen pharmaceutical legal departments. A biosimilar wins FDA interchangeability designation — months of clinical switching studies, millions in development costs — and then a carton labeling discrepancy quietly puts that designation at risk during an FDA inspection. The product name, the 4-letter suffix, the reference product citation: any of these, rendered incorrectly across labeling components, can produce a Form 483 observation that unravels the entire commercial story.

FDA’s Purple Book database tracks every licensed biological product and marks which ones carry interchangeable status. That database doesn’t just inform pharmacists and payers. It informs FDA investigators, too. And during a biosimilar labeling audit, it’s one of the first places an investigator goes.

What “Interchangeable” Actually Requires on a Biosimilar Label

The Biologics Price Competition and Innovation (BPCI) Act of 2009 created two tiers of biosimilar recognition: biosimilar and interchangeable. The distinction matters enormously for labeling. Under Section 351(k) of the Public Health Service Act (42 U.S.C. 262(k)), an interchangeable biosimilar has met an additional evidentiary standard — specifically, that switching between the biosimilar and its reference product produces no greater safety or efficacy risk than continued use of the reference product alone.

Pharmacists in more than 40 U.S. states can substitute an interchangeable biosimilar for a reference product without first contacting the prescriber. That’s a significant commercial advantage. It’s also a labeling liability if your documents don’t handle it precisely.

FDA’s 2019 guidance, Labeling Recommendations for Biosimilar Products, specifies that the prescribing information must:

  • Identify the reference product by proper name and the biosimilar by its own nonproprietary name — including the FDA-designated 4-letter distinguishing suffix (e.g., filgrastim-sndz)
  • Accurately reflect the conditions of use for which biosimilarity or interchangeability was demonstrated, without extrapolating beyond the evidence base
  • Refrain from comparative effectiveness or superiority claims relative to the reference product unless those claims are supported by substantial evidence and included in approved labeling

That last point is where most violations cluster. Marketing teams see “interchangeable” as a quality signal they want to amplify in every channel. Regulatory affairs teams know it’s a precisely scoped FDA designation — not a general superiority statement — and the two perspectives don’t always resolve cleanly before materials reach distribution.

The Five Labeling Elements FDA Scrutinizes Most in Biosimilar Audits

Over roughly the past five years, CDER and CBER have flagged a consistent set of biosimilar labeling issues in warning letters and 483 observations. If your audit program isn’t checking each of these systematically, you’re leaving measurable exposure on the table.

1. Nonproprietary name accuracy across all label components. FDA finalized its naming policy for biological products in January 2017. Every biosimilar must use its FDA-designated 4-letter suffix — and that suffix must appear consistently across all labeling components: the carton, the immediate container label, the prescribing information, and patient labeling materials. A single component that drops the suffix creates a discrepancy that FDA investigators actively look for.

2. Reference product citation currency. The label must correctly identify the reference product by its current approved name. If the originator brand has undergone any labeling updates since the biosimilar’s approval, the biosimilar sponsor may need to assess whether a labeling supplement under 21 CFR Part 601 is warranted. Many sponsors don’t have a systematic process for tracking originator label changes — and they discover the divergence during inspection, not before.

3. Interchangeability designation language. FDA permits biosimilar labels to state the interchangeable designation, but only when the product actually carries it in the Purple Book. Labels that imply interchangeability through indirect language — phrasing like “can be substituted for” without a proper Purple Book-confirmed designation — have drawn Office of Prescription Drug Promotion (OPDP) attention. With more than 60 biosimilars approved as of 2025 and roughly 12–15 carrying formal interchangeable status, the Purple Book cross-reference is a mandatory audit step, not an optional best practice.

4. Indication carryover from the reference product. Biosimilars don’t automatically inherit every indication held by the originator. A biosimilar approved only for certain conditions cannot carry labeling language that implies coverage of the full reference product indication set. This error is surprisingly common in multi-indication biologics where the biosimilar sponsor pursued approval for a subset of indications and the labeling language was never scoped accordingly.

5. Carton-to-PI version consistency. Spot-check any 20 biosimilar carton samples against the current approved prescribing information and you’ll find at least a few with version lag — the carton reflects an earlier iteration of the PI. Version control is governed by 21 CFR Part 211 on the manufacturing side, but the audit finding shows up as a labeling compliance failure. It’s one of the most consistent 483 sources in product labeling reviews, and one of the most preventable.

Your own label may require a supplement under the Changes Being Effected (CBE) or Prior Approval Supplement (PAS) pathway when certain content changes are made. A durable audit program has four core components.

Quarterly Purple Book reconciliation. Assign clear ownership for cross-checking every approved biosimilar in your portfolio against the current Purple Book. The specific items to verify: interchangeability status (added or removed), reference product withdrawal notices, and new biosimilar entrants that could affect your approved indication scope or market positioning language.

Trigger-based labeling change monitoring. When the reference product sponsor submits and receives approval for a labeling supplement, you have an obligation to assess whether your biosimilar labeling needs to follow. Automated monitoring tools can alert your team within days of a reference product PI update — rather than discovering the divergence at your next scheduled periodic safety report or, worse, at an FDA inspection.

Pre-promotional review for interchangeability claims. Any promotional material — print, digital, HCP-directed, or direct-to-consumer — that references interchangeability should route through a review step that confirms current Purple Book status at the time of distribution. Designations aren’t permanent. A promotional piece developed when your product held interchangeable status could remain in circulation after a status change, creating exactly the kind of misbranding exposure that OPDP warning letters are built around.

21 CFR Part 601 supplement tracking. Every labeling supplement your organization submits should be tracked in a system that links the approved supplement to the specific labeling components it governs. This creates an auditable revision history — critical when an FDA investigator asks to see the documentation trail for a particular label element.

The Practical Upshot

Biosimilar labeling compliance sits at an uncomfortable intersection of biological product regulations, naming guidance, promotional rules, and a living database that updates continuously. None of those inputs are static. The reference product’s label changes. FDA confers new interchangeable designations. State substitution laws evolve.

No purely manual audit process refreshes quickly enough to track all of those simultaneously — and that’s not a criticism of the teams doing the work.
